    The Resource Center offers a variety of primary medical care, dental care and health-related services, all of which are available to the entire community at the TRC Community Health Center. The Primary Care Clinic, located in Jamestown, features board-certified physicians with expertise in treating people with complex health issues. Dental Clinics, located in Dunkirk and Jamestown, provide quality dental care, particularly those unable to access dental services elsewhere. The agency’s Diagnostic & Treatment Clinics, located in Jamestown and Dunkirk, offer occupational therapy, physical therapy, podiatry, and speech/language therapy.

    Note: No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.  Please contact us for for more information at 716.661.1447.

    TRC Community Health Center provides a whole array of services for each primary care patient.  TRC Community Health Center has been serving the community for close to 30 years.

    Many people in our community utilize the TRC Community Health Center for their health care needs. Our Community Health Center, located at 890 East Second Street in Jamestown, provides a comprehensive level of care to people of all ages. People from all walks of life count on the experienced medical staff at TRC Community Health Center. We are able to bill most insurances, and we are one of the few medical practices in the area that accepts Medicaid. Our accreditation as a Level 3 Person Centered Medical Home is recognition of the high-quality services that TRC Community Health Center provides, and it means we will give you the best care possible while making sure you have a clear understanding of YOUR personal health.

    Dr. Adnan Munir, MD

    Dr. Adnan Munir is The Resource Center’s Medical Director.  He has been with TRC since 2008.  He is board certified in internal medicine and focuses on family medicine, including caring for children.  Dr. Munir is a member of the American Academy of Family Practice, American Medical Association, and Spanish American Medical Association.  He has been cited as one of Western New York’s outstanding physicians by Castle Connolly Medical, a health care research and information company that seeks to guide people to America’s top doctors and hospitals.  He speaks English, Spanish, Urdu, Punjabi, and Hindi fluently.

    TRC Community Health Center Primary Care Scope of Services

    • Behavioral Health & Dental Services are available on site at TRC Community Health Center.
    • Preventative Health Services are offered to any primary care patient and each is encouraged to complete evidence-proven preventative screening.

     

     

    TRC Community Health Center offers:

    • Same-day appointments, including new patients
    • Acceptance of most insurances
    • Plenty of free, off-street parking
    • Ability to transfer medical records from a previous provider

    Patient Information

    Please review the patient information below before attending your appointment.

    Making an Appointment:

    • You may make an appointment by contacting us at:  716-661-1447, or you may use your patient portal to request an appointment. 
    • When you’re sick, calling our office early in the day will help us schedule you a same-day visit.  Our front-desk staff are available 8:30-5:00 Monday through Friday to answer your questions and to make an appointment.
    • If your address, phone number, or insurance information has changed, please let us know when making your appointment, so that we have the most up-to-date information for you. 
    • Please come to your appointment prepared with the reason for your visit.  If you are a new patient, with a complaint – please notify us at your reminder call that you have a health-related concern that you would like to discuss at your new patient appointment. 

    When You Arrive:

    • Please arrive at registration window “A” at least 15 minutes before your appointment to allow time before your appointment to complete your registration, verify insurance information, and to complete any additional paperwork that may be needed for your record.   
    • Insurance cards and a valid photo ID are required for your appointment 
    • If you do not have insurance coverage, please be prepared to pay for your visit at the time of service.  Our Patient Access Representative will be happy to help you determine the cost of an uncovered service, or any copayments due, prior to arrival. 

    Appointment Cancellations:

    • TRC has a (3) THREE no-call/no show policy.  We kindly request that you notify us 24-hours prior to your appointment of a cancelation.  Calling before your appointment allows us time to make that appointment available to someone who may be sick, or need urgent care.  You may also call the Primary Care office after-hours to cancel an appointment, and our answering services will notify us of your cancellation. 
    • Under certain conditions, the patient may be prevented from scheduling future appointments, but not from receiving care. Patient will sign acknowledgement of the no show process during patient registration, and/or with any practice updates.

    Missed Appointments:

    First Missed Appointment: The patient will be re-scheduled on the provider’s schedule, and sent a letter to be informed that they have missed ONE appointment, and that TRC Community Health Center carries a policy that three consecutive missed (no Show) appointments will result in a patient being placed on our Providers schedule with a tentative appointment time to be worked into the providers schedule which could result in extended wait times to be seen by a patients provider.

    Second Missed Appointment: The patient will be placed on the provider’s schedule, and sent a letter to be informed that they have missed TWO consecutive missed (no Show) appointments, and that TRC Community Health Center carries a three no-show policy.  Patients who chronically no-show will result in a patient being placed on our Providers schedule with a tentative appointment time to be worked into the providers schedule which could result in extended wait times to be seen by a patient’s provider.

    A Patient Access Navigator may also reach out to you at this time, if you are having trouble making your appointments. 

    Third Missed Appointment:  The patient will be sent a letter to be informed that they have missed THREE consecutive appointments without cancelling in advance. TRC Community Health Center has a 3 No Show appointment policy which has altered their ability to schedule appointments with your provider. At this time, we can only provide you with an appointment at a tentative time. Once you arrive for your appointment, you will be worked into the provider’s schedule which might lead to an additional wait time.

    Patients who incur THREE missed (no Show) appointments may be subject to dismissal from the practice.   The patient’s chart will be reviewed, and dismissal will be determined by the Physician and Practice Manager only.  

    Once a patient has kept two consecutive appointments, they may return to the regular scheduling process.

    Appointment Reminders

    • It is extremely important that you notify your office of any phone and/or address changes. Automated reminder calls/texts will be sent to your number of preference 1 week, 3 days, and 1 day prior to your scheduled appointment.  Please acknowledge the reminder with an appropriate response.
    • A reminder call directly from our office will be provided approximately one week prior to your appointment, to ensure that any required lab work, referrals, or procedures have been completed prior to your appointment, if needed. 
    • A final reminder call directly from our office is given the day prior to your appointment, if you have not responded electronically.  If you do not confirm your appointment, we are to assume that you are unable to make this appointment, and the appointment may be opened up to another patient.  If you arrive to your appointment without confirmation, or you arrive late for your appointment, you may or may not still be seen that day, depending on the provider’s availability.  

    Medications & Refills 

    • Please allow 72 hours to process refill on any prescription

    • Call your pharmacy first to refill your medications.  The pharmacy will contact our office directly if a new prescription is necessary.

    Please bring all medications with you (even those not prescribed by us), when you come for your appointment.  If you are diabetic, please bring with you, your blood sugar logs, glucose meter, and all diabetic medications. It is extremely important that we maintain accurate medication records & data for continuity of care.

    Therapy Services

    Physical therapy is a field of rehabilitation in which therapists work with people to accomplish goals that help a person return to a normal level of activity.

    TRC’s clinics have physical therapists and physical therapy assistants with more than 60 years of combined experience. This enables the clinic to effectively treat outpatients from the community with various injuries.

    TRC Physical Therapy also utilizes the Biodex Balance Assessment Program for Concussion and Fall Risk Screening and Conditioning. The Biodex system brings together cognitive and functional objective assessment to determine a persons risk for falling. The Biodex system also has been utilized to provide an accurate assessment tool for concussion management.     Cain Sonney is a Physical Therapist at our Diagnostic and Treatment Centers Jones & Gifford Avenue in Jamestown.  Cain obtained his doctorate of physical therapy from Gannon University, where he also received his bachelor degree in sport and exercise science.  Cain was drawn to explore a career as a physical therapist because of his experiences in the Armed Forces. While working on his doctorate, Sonney did one of his clinical rotations in The Resource Center’s physical therapy department on Jones & Gifford Avenue in Jamestown.  While there, he interacted with a number of people with developmental disabilities who participate in the day habilitation program located in that building.  His experience at TRC led him to seek a full-time position at the agency. Cain likes working one on one with his physical therapy patients and helping them reach their full potential.

    Occupational therapy assists people in developing the “skills for the job of living” necessary for independent and satisfying lives.     Adaptive Equipment Therapy Services

    Adaptive equipment is any device, tool, or machine that is used to assist with any daily task. Adaptive equipment in occupational therapy helps those with short-term or long-term disabilities where they may be having trouble performing their normal routines.

    Adaptive equipment is important in occupational therapy because in order to truly improve the quality of one’s life, giving them as much independence as they’re capable is key.

    School-Based Health Center

     

    A School-Based Health Center is an expanded school health program provided by a joint effort between the school district and The Resource Center. A School-Based Health Center (SBHC) provides comprehensive health care services to students enrolled at the school. For children who already have a health care provider, the SBHC staff can work with that provider in caring for your child. For children without a primary health care provider, the SBHC staff may become the child’s primary source of health care.

    The School Based Health Center at Jamestown High School also provides dental hygiene services. The SBHCD clinic doesn’t take the place of your family’s dentist, but instead works as an additional service to enhance the dental care of the student in a convenient location. Your family dentist will always be informed of any treatment or problems. If you don’t have a dentist or insurance, the SBHCD will work with you to determine what options are available. Nobody will be turned away because of an inability to pay and parents will always be contacted prior to any treatment.

    When the school is not in session, PLEASE call your primary care provider.  If you do not have a primary care provider, please call 716-661-1447.

    Dental Hygientist services provided to students include:

    • Dental Cleanings
    • Digital X-Rays
    • Fluoride Treatments
    • Dental Sealants
    • Ability to email x-rays to family dentist.

    The School Based Health Center at Jamestown High School also provides mental health services for students who are in need of these professional-based services. Today’s student population can face a lot of pressures and emotional stress not only from the demands of school but from peer pressure. It’s important to seek help and medical attention when needed. The School Based Health Center at Jamestown High School provides these important services on site.

    School Based Health Center Staff

    Nicole Hinderleider, NP

    Nicole Hinderleider, NP is a Family Nurse Practitioner at the JHS school-based clinic. Nikki received her Associates Degree in Nursing from Jamestown Community College and her Masters of Family Nursing from the University of Toledo.   Nikki joined the JHS school-based clinic in 2023. Nikki always knew she wanted to work in the health care field from an early age. She worked in a nursing home at the age of 16 and then pursued a position in medical records at UPMC Chautauqua.  She joined The Resource Center to fulfill an opportunity to work with high school students.  Nikki truly enjoys working with high school aged children.  Her goal is to help students engage more with the health care system and be an available resource to help them succeed.

    Lauren Consiglio, RN

    Lauren Consiglio, RN is a registered nurse at the JHS school-based clinic. Lauren received her LPN from the Hewes Educational Center in 2010 and her RN from the Hamot School of Nursing in 2022.  Her extensive background includes pediatric nursing at Jamestown Pediatrics and serving as the Nurse at Camp Onyahsa.  She has over 13 years of experience.  Lauren joined the JHS school-based clinic in 2023. Lauren loves helping and advocating for others and truly enjoys making her community of Jamestown a better place.  She joined The Resource Center to fulfill a desire to work with school-aged children and make a difference in their lives.  Lauren is also excited to be a part of our newest school based health center at Washington Middle School.

    Kristy Drake, Patient Access Representative

    Kristy Drake is a patient access representative (PAR) at the JHS school-based clinic. Kristy earned a Bachelor of Science in Business Management.  She joined the JHS school-based clinic in 2022. Kristy loves working with kids and enjoys her position in an office setting.  She decided to join TRC's School-Based Clinic because of the values of the organization as well as the room for career growth.
