To serve as an active member of the Interdisciplinary Team; To promote and enhance person-centered outcomes; To serve as medical coordinator/advisor/liaison on behalf of consumers medical needs/interests in conjunction with family member wishes and community provider recommendations; To ensure consistent compliance with established treatment plans; To serve as a mentor to LPNs and recent graduates.
