    • HR – Specialist

      To assist and facilitate various functions within area of responsibility.

    • Employment Case Manager

      To partner with persons receiving supports by developing a comprehensive, integrated, and clear strategy for the TRC habilitation services that support them; ensuring cohesive implementation and focused progression towards each person living a full and satisfying life commensurate with their lifestyle and realizing their life goals and citizenship with urgency, intention, and determination. To co-coordinate care for each person’s individualized plan with Care Coordination.

    • Speech Pathologist

      Will assess and evaluate individuals’ needs and develop and/or implement service plans to meet those needs.  Comprehensive knowledge and skills to develop and implement a wide variety of therapy techniques, strong assessment skills and the ability to develop service plans to address individuals’ needs. Strong verbal and written communication skills. The ability to self-direct and self-initiate, and handle a specified caseload.

    • Mental Health Clinician

      The Mental Health Clinician II will provide direct input in the assessment and program planning for individuals assigned to their caseload. The Clinician will provide a wide variety of direct service client contact on an ongoing basis.

    • Maintenance Specialist

      Able to effectively perform work in the following trades: electricity, carpentry, plumbing, and general maintenance with a high degree of proficiency.

    • Assistant Residence Manager 3

      To assist the Residence Manager in the effective operation of the residence. In conjunction with and/or in the absence of the Residence Manager, provides supervision to direct care staff and ensures the implementation of individualized programming for each individual who resides at the residence.

    • Licensed Practical Nurses (LPNs)

      To assist in the provision of delegated medical/nursing services. This includes the training of staff in medical/nursing procedures, completion of required documentation and communication with physician. May perform CLS responsibilities as requested by the Residence Manager/Site Supervisor.

    • Direct Support Professional

      To provide direct care and supervision in all activities of daily living, implement individual’s Habilitation Plan, complete required documentation regarding individual milestones and behavior management programs, participate as an active member of a team with other program/department staff to provide the identified supports to participants, and enhance the skills and/or create the opportunity for consumers to achieve maximum independence, inclusion, individualization, and productivity.

    • Community Coach

      Provide direct individualized support services to service recipients to enable them to lead self-directed lives in their homes and to contribute to and be included in their communities in an ethical, holistic and professional, non-judgmental manner that builds upon the recipient’s strengths, choices and capabilities.

    • RN – Residential

      To serve as an active member of the Interdisciplinary Team; To promote and enhance person-centered outcomes; To serve as medical coordinator/advisor/liaison on behalf of consumers medical needs/interests in conjunction with family member wishes and community provider recommendations; To ensure consistent compliance with established treatment plans; To serve as a mentor to LPNs and recent graduates; To promote a life of hope, dignity, and purposeful living for all.
